About

Diabetes Partnership of Cleveland generates the resources that this chronic disease implie for those living with type 1 or type 2 diabetes.

Diabetes Partnership of Cleveland is the go-to resource for people looking for immediate and long term support for managing this chronic disease in the total sense, outside their medical home. Not only do we provide tools and education for diabetes self-management, we address the needs of the person as a whole - physical, emotional, and social.

We have a program for children with diabetes, Camp Ho Mita Koda, located in Newbury, Ohio. This special camp gives children with diabetes the opportunity to share experiences and learn vital self-care skills while enjoying all the fun, outdoor activities that make unforgettable summer memories.

Nestled in 72 wooded acres in Newbury, Ohio, just 25 miles east of Cleveland, Camp Ho Mita Koda combines  the highest quality, nurturing medical care and diabetes education with fun, outdoor adventures and fast friendships. Activities include swimming, kayaking, canoeing, horseback riding, archery, arts & crafts, drama, team sports, and much more.

From the increased self-esteem, sense of independence, and getting to feel just like any other kid, to the friendships that last a lifetime, Camp Ho Mita Koda will always remain a special part of the lives of the children it has touched.  Happy Campers Make Happy Parents!

The ultimate goal of Diabetes Partnership of Cleveland is to prevent or delay the many complications related to poorly managed diabetes.
