We are a nonprofit organization with more than 35 years of experience helping adults with long-term care needs live as independently as possible. Our Medicare/Medicaid long-term care programs serve the needs of older adults and adults with disabilities. We coordinate and deliver a full range of supportive services that help them live safely, confidently and with dignity.
More than 10,000 Wisconsinites trust in us to provide the quality support needed to help them reach their potential in health, happiness and independence. We are Wisconsin-based and have local offices supporting each of the 14 counties we serve.
Community Care's mission is to provide quality long-term care and member-centered approaches to support its members, so they can live functionally independent in their communities. Community Care provides a wide variety of long-term care and health services to seniors and adults with disabilities in 14 counties: Milwaukee, Waukesha, Calumet, Fond du Lac, Kenosha, Manitowoc, Outagamie, Ozaukee, Racine, Sheboygan, Walworth, Washington, Waupaca and Winnebago Counties. Our Care Teams manage care and services to individuals in their homes and communities. We also operate Adult Day Health Centers in Milwaukee and Waukesha counties, which serve as both health clinics and adult day centers. Because our programs are funded by Medicare and Medicaid, many of our members who are enrolled in both pay little or nothing to receive these exceptional health care services.
