The Mississauga Halton Community Care Access Centre provides clinical assessments, advises and manages its diverse clients' need to live safely at home or help them transition to long-term care.  The MH CCAC serves more than one million residents in South Etobicoke, Mississauga, Oakville, Georgetown, Halton Hills and Milton.

All CCACs are funded by Ontario's Ministry of Health and Long-Term Care, through designated Local Health Integration Networks (LHINs), at no direct cost to clients.

CCAC serves:
. More than 39,000 clients in 2011-12, with more than 2.1 million visits
. A highly diverse area with a 43.2% immigrant population (compared dto 28.3% provincial average)
. One of Canada's fastest growing population of seniors in Canada - with a projected 32.3% increase in 75 to 84 year-olds and 71.1% increase in seniors 85 and older by 2013.
