Senior Health Information Network is an in-home care provider serving senior populations across the Northwest regional states. Senior Health Information Network offers non-medical services in the home through a holistic care model and Medicare-certified home care network services. Our clients can live safely in the comfort of their homes – even with complex medical situations – we evaluate every aspect of our clients' health — medical, functional, psychosocial, emotional and spiritual. We implement care consistently and professionally. Our clients remain at home and out of the hospital and emergency room. We monitor relentlessly, provide consultation with their doctors, share information with only appropriate providers, and constantly tailor care to meet their changing needs and preferences for the client and caregiver. Both are important to us and we look to meet the needs of each one. We keep our clients at home and only when necessary will we provide 24-hour medical homes for seniors at their request. We serve a major important population in the health care industry. We concentrate our skills and experience to create a health care network organization dedicated to providing quality care, advocacy, and services that reveals dignity and compassion for seniors and caregivers. All our staff and services are built on a strong foundation of individualized attention for each client. Our consultants are highly experienced. We understand that every health care need is different. We take the time to understand your unique needs and develop a comprehensive plan to fit your business.

Senior Health Information Network also is recognized as a national provider through the National Diabetes Prevention Program which is authorized by the Centers for Disease Control. Our national prevention effort is to assist in organizing and building public-private partnerships such as employers, organizations, faith-based entities and communities to offer evidenced-based low-cost interventions in communities across the United States to prevent type-2 diabetes.
