We implement care consistently and professionally. Our clients remain at home and out of the hospital and emergency room. Senior Health Information Network also is recognized as a national provider through the National Diabetes Prevention Program which is authorized by the Centers for Disease Control. Our national prevention effort is to assist in organizing and building public-private partnerships such as employers, organizations, faith-based entities and communities to offer evidenced-based low-cost interventions in communities across the United States to prevent type-2 diabetes.
